What You Need to Know

Getting certified takes five steps. Your instructor will guide you through signing up for your participation day during the course.

Don't Miss This

Submit your affidavit and $40 fee to the Dale Foundation within 60 days of completing the DANB RHS Review Course.

Your Step-by-Step

  1. Attend a 2-hour participation day. Print the Affidavit of Ohio Clinical Radiography Training and bring it with you. If you don't attend one during the course, you'll need an Ohio-licensed dentist to sign your affidavit.
  2. Complete the DANB RHS Review Course. Self-paced and online through the Dale Foundation ($130). Score at least 75% on the post-course assessment.
  3. Submit your affidavit. Within 60 days, send it with the $40 processing fee to the Dale Foundation by email, mail, or fax (see the bottom of the form).
  4. Watch for confirmation. In about 3–4 weeks, the Dale Foundation emails you and notifies the Ohio State Dental Board. Save that email.
  5. Apply with the Ohio State Dental Board. Apply online under the "Initial Application – General" tab ($32), and include the Dale Foundation email.
